
Continuing Your Journey After Treatment with Help From OHC
At OHC, we understand that cancer treatment can impact your health and your life even after treatment is completed. Your OHC care team will help you with the transition from treatment to survivorship.
At the end of your treatment, we will schedule a survivorship visit with an advanced practice provider. They will discuss with you the following:
- A personalized follow-up care plan schedule with information about the next steps after treatment and goals for achieving wellness
- Monitoring and managing ongoing side effects and symptoms, and assistance to make them more tolerable, with the ultimate goal of relief
- Any maintenance or ongoing medications that you will continue after your cancer treatment that may be a part of your follow-up care
- How to improve your health through nutrition, exercise, weight management and smoking cessation
- Counseling and emotional support
- Fatigue management
- A copy of your survivorship plan will be provided to you and shared with your primary care doctor
Completing treatment is a major milestone, but it’s important to keep an eye on your health and monitor for any changes or new symptoms. You will continue to see your OHC care team periodically. The appointments will be more frequent at first and become less frequent over time. During these visits, your care team will:
- Review of your medical history
- Perform physical exams
- Help with any treatment-related problems that persist after your treatment is completed
- Recommend appropriate cancer screenings
- Discuss how to ease back into normal life after treatment including returning to work
